La Solana Care and Rehab

From the gorgeous, enclosed courtyard that provides space for family to gather, to nearby shopping and motels, to the bilingual staff (someone is on site at all times who speaks both English and Spanish) – La Solana is dedicated to family-centered care. We view the quality of life of our residents as our responsibility and know that our residents are happiest when family members participate. We encourage you to visit, take part in planning the care of your loved one, and participate in the care that is given.

Short-Term Rehabilitation

At La Solana, therapists employ an individualized and holistic approach to treatment. This begins with the design of a rehabilitation plan – a combined effort by the patient, the La Solana therapy team, and other members of the patient’s care team (such as family members and physician). Such a plan may include physical and/or occupational therapies and is focused on helping a patient regain independence and return to activities previously enjoyed.

Long-Term Skilled Nursing Care

La Solana’s skilled nursing team is dedicated to preserving the dignity of our residents and helping them maintain as much independence as possible. Care given and therapies employed are individualized and focused on helping a resident maintain his or her highest level of functionality. Services offered include:

Respite Care

Over 50 million people in the United States are providing in-home, full-time care for elderly loved ones or other family members with disabilities and/or chronic illnesses. Caregivers in such situations need to take time to emotionally and physically recharge. La Solana’s staff will care for your loved one for a short period of time, in our facility, while you take a vacation, deal with other responsibilities, or simply rest and recover.

Hospice Care

Hospice Care involves the efforts of the La Solana interdisciplinary care team to provide for the needs of the resident and his or her family members when end of life is near. This very special form of care is focused on relieving the resident’s symptoms, especially pain, and on providing social, emotional, and spiritual support.
